CENTRAL ACCESS SERVICES
This central access referral service provides a single point of entry for patients who require assessments and pain management and are referred to by clinicians.

ELIGIBILITY REQUIREMENTS
  • Standard Referral form is complete
  • Referral accepted from Medical Physician, Nurse Practitioner or Dentist
  • Patient must be under the care of a family physician
  • Patients must be 18 years of age or older
  • The client’s case is complex and involves multiple programs or diagnoses that have been satisfactorily investigated.
  • Single discipline treatments have not yielded significant improvement in function and any of the following exist:
    • The patient describes ongoing or recurrent pain and discomfort that affect their physical, psychosocial, and/or vocational function
    • The patient describes ongoing or recurrent psychosocial issues and stress that affect they physical, mental and /or vocational function
  • Length of time from illness and/or injury has been greater than 6 months.
    • Exceptions include emerging conditions such as Complex Regional Pain Syndrome. These cases should be referred as soon as possible.
  • Out-of-province referrals will be accepted at UAH Multidisciplinary Pain Clinic and LifeMark Health Institute. Active Alberta Health Care required for Lifemark Health Institute.
  • Patients cannot be assessed while inpatient
  • Incarcerated patients - Correctional Service of Canada: Please send referral directly to a Chronic Pain Clinic. AHS cannot accept referrals for incarcerated patients

Inclusion Criteria:
  • have complex chronic pain
  • are 18 years of age or older
  • reside in Alberta with a valid Alberta Health Care number
  • have a primary care provider prepared to work closely with the treatment team and provide follow-up
  • are in stable medical condition
  • are cognitively capable of participating in assessment and treatment
Exclusion Criteria:
  • no Emergency Room or inpatient referrals.
  • pregnancy
  • have a mental health condition that would preclude participation in assessment and treatment
  • have an untreated, unstable substance addiction
  • have an active cancer diagnosis

Alberta Wide
Alberta Virtual Chronic Pain Program -
A virtual group education treatment program over Zoom for people in Alberta experiencing persistent pain longer than 3 months.
For patients looking to better understand their pain, connect with others living with pain, and seeking strategies on managing pain beyond the use of medications. An interactive program led by health providers with peer support incorporated throughout, as well as post-completion follow-up for up to 1 year. This program offers:
• a 1-hour session each week for 6 weeks (virtual group education and self-management program)
• highly encouraged 1-hour peer support group sessions each week
• opportunity to connect with others living with pain
This program doesn’t offer:
• diagnosis
• medical intervention
• one on one care
• prescription advice or renewals
A diagnosis is not required to access this service. Self-referrals can be made by calling the toll-free number, 1-877-719-7707

Allied Health Chronic Pain Education Sessions -Introduction to Chronic Pain is a 3-hour education session facilitated by CRIS staff in a group setting.  The program will provide clients and caregivers with an understanding of chronic pain and factors that impact and influence pain; an introduction to self-management strategies; and information regarding community resources available for clients experiencing chronic paid.  An early understanding of the general principles of chronic pain will support clients in accessing appropriate, available community resources. To register call:  (780)342-3385

Chronic Pain Self-Management Better Choice, Better Health Chronic Pain Self-Management Program is a 6 week workshop series that provides information and tools to assist patents in managing a chronic pain condition.  Workshops are held at various location across the Edmonton Zone - To register call:  780-735-1080 ex. 3

Pain Rehabilitation Education Program (PREP)PREP is a 6-8 week pain education and self-management program for clients with chronic pain.  The program involves 1:1 sessions with a physiotherapist or therapy assistant teaching people about pain and strategies to support positive life changes.  Exercise is also included as a key component of this program. For clinic and/or referral information, please call:  (780)342-3385

Community Rehabilitation Interdisciplinary Services (CRIS)The CRIS Program provides client-centered, goal-oriented, time-limited active rehabilitation services to maximize a client's functional independence and increase participation in daily life. The primary purpose of the All Locations list is to let the user easily access any location of a healthcare service without going back to the main search screen.

The locations listed have 3 background colors:
  • Green means the healthcare service@location has referral information attached to it.
  • Brown means the healthcare service@location never had referral information attached to it, or it has unpublished referral information.
  • Red means
    • IA changed the healthcare service@location's status to something other than Current
    • It was deleted if it is an ARD healthcare service@location.
Green  and Brown are always at the top of the list. These are the Healthcare Service@Locations with the status of Current.
The Red list at the bottom consists of non-current Healthcare Service@locations that once had Published referral information in the ARD.
If the referral information was never published in ARD the Healthcare Service@location will not show in the Red list.

The secondary purpose of the All Locations list is to allow ARD Administrators to recover (copy) referral information from the non-current Healthcare Service@Locations to ones that are current.

Common Scenario:
A Healthcare Service moves from one location to another. In this case the IA Healthcare Service@Location record will be made defunct (non-current) and a new Healthcare Service@Location record will be created with a current status. In this scenario the captured referral guidelines in ARD can become "orphaned" as they are not attached to any current IA healthcare service.

Categories of non-current or orphaned referral guidelines: INDIVIDUAL and COMMON.
The REFERRAL GUIDELINES section of the profile has the prefix INDIVIDUAL or COMMON to help you choose the method below when transferring referral guidelines from a non-current Healthcare Service@Location to a current healthcare service@location.

Individual referral process
  1. Click on a non-current (Red) Healthcare Service@Location at the bottom of the All Locations list.
  2. The non-current referral info is displayed with the link Copy this Referral Process to another Healthcare Service@Location link on the upper right hand corner. Click on the copy link.
  3. Choose a current location (Green or Brown) from the All Locations list. This will be the Healthcare Service@Location you are pasting the referral info into.
  4. The system will display the Edit Referral Info screen populated with the referral info from the non-current Healthcare Service@Location you viewed in the first step.
  5. Click Save and the referral info is transferred from the non-current Healthcare Service@Location to the current one.
  6. Repeat these steps for each Healthcare Service@Location that needs attention.

Common referral process - 2 sub cases.
Case 1: At least 1 current Healthcare Service@Location with common referral info is with current status for this healthcare service; One or more Healthcare Healthcare Service@Locations where replaced by new one.
  1. Click on any current Healthcare Service@Location whether it has referral info (Green) or not (Brown).
  2. The healthcare service location opens in the Edit Referral Info screen populated with the current common referral info.
  3. Save it. 
  4. All locations will be updated with the common referral information, including all the locations that don't have referral info yet (Brown). The non-current referrals (Red) will also be updated.
Case 2:  All Healthcare Healthcare Service@Locations for a healthcare service are set to a non-current status and replaced by new ones. In this case there is no current additional referral info to copy from, so the only alternative is to pick up the non-current common referral process (Red). Follow the steps described in the section Individual Referral Process above to copy/paste the non-current common referral info to the current healthcare service locations.
Generally we want to replicate current common referral info to new or replaced healthcare service locations. We only resort to copying non-current common referral info if there is no other option.

Remember: Some fields can be location specific with the common referral process:
Parking Instructions, Directions, Parking Map, Wait Time, Referral Phone or Referral Fax.
To update these items you have to edit each Healthcare Service@Location separately.

ADDITONAL NOTES:
  • The info icon after the All Locations drop down will be visible to ARD Administrators.
  • The system doesn't allow you to copy referral information from one non-current Healthcare Service@Location to another.
